****All new player registration will be subject to space availability and space cannot be confirmed until documentation is received and approval received by BC Hockey/Hockey Canada. This process may take several weeks or up to 2 months so please check the requirements carefully and submit all documents asap****

If born in CanadaPCAHA New Player Registration FormPlayer Movement Form (if player played previously) | New Players - 2 Proofs of Residency (ex: Utility bills (hydro, gas, water), cable/satellite, property tax bill/assessment -property/contents insurance -bank/visa statement, BC medical statement -GST statement or income tax assessment, School registration confirming address) Birth Certificate (Copy is okay) If Born outside Canada, email the office as there is documents we require.
Player Movements -3 Proofs of Residency -utility bills (hydro, gas, water) - preferred (we require the complete first page, including pay stub -cable/satellite/internet -property/contents insurance -property tax bill/assessment -car insurance/driver’s license -bank/visa statement -GST statement, income tax assessment, child tax credit -School registration confirming address |
