THIS APPLIES TO ALL RECREATION AND REP TEAMS!!!

Chilliwack Minor Hockey Association’s jersey and pant sheet (U13-U21) deposit amounts are as follows:

• U11 $75.00 (1 jersey only and no pant shells)
  • Keep black jersey this year
• U13 $200.00
• U15 $200.00
• U18 $200.00
• U21 $200.00

Please note that the pant shells will need be returned to CMHA at the end of each season just like player jerseys 

1. Team Managers are requested to complete the Jersey Deposit Tracking form as follows:
   1. Complete form as deposits are received from parents.
   2. Collect deposits in the form of cheques, credit card authorization or cash for all jerseys.
   3. Submit form and deposits to the office prior to November 1^st, 2023. Contact Sara Pudwell at info@chilliwackminorhockey.com to arrange an appointment prior to the deadline.
   4. Any team that has not submitted their Jersey Deposit Tracking form and deposits by November 1^st, 2023 will be charged for all jerseys.

Accepted Deposit Types:

Cheques MUST be:

1. Made payable to: CMHA
2. Memo: Player’s name and jersey #
   1. Postdated: March 31st, 2024

Please Note: Any cheque that does not follow the above guidelines will not be accepted.

Preauthorized Credit Card Form:

1. Attached

Cash:

1. Accepted in envelope with jersey #, player name & parent name.

NOTE: All players MUST have a Jersey and Pant Shell Deposit on file

2. All players will have the opportunity to purchase their jerseys.
   1. Please find the Jersey Order form attached.
   2. Submit this form to the office no later than February 1^st, 2024
